Home Infusion Pharmacy Manager September 2014 to August 2021
Obtained buy-in from pharmacy, nursing, and physician partners to implement a full-service Home Infusion Pharmacy at the Kaiser Antelope Valley Area. Built relationships with Long Term Facilities to arrange pharmacy managed antibiotics, narrow therapeutic medications, medication reconciliation and anticoagulation for Kaiser Members in those facilities. Educated Case Managers and Hospitalists about the new services and managed the program with the help of the Pharmacist-in-Charge. Coordinated patient care alongside the Director of Case Managers and Home Health. Rolled out pharmacist training and a new platform for this new service. Provided antibiotic stewardship services to Infectious Disease. Established a contract with CAPs to receive complicated Total Parental Nutrition. Developed a Clinimix discharge program to improve patients’ satisfaction. Accountable for the rate of hospital re-admissions, length of stay and developing programs to manage these metrics. Conducted quality assurance such as rate of line infections, number of patients being at goal with medications that possess a narrow therapeutic range. Responsible to examine pharmacist and technician competencies, continuation in practicing within a professional scope, knowledge of policies/guidelines. Reviewed, wrote, modified, and implemented Home Infusion Polices. Participated in survey readiness meetings and conducted regular rounds to ensure that the area is survey ready. Created policies, guidelines and required documents by ASHP to implement the first residency program at Kaiser Permanente Antelope Valley. Drug Utilization Management Coordinator December 2007 to August 2021
Managed the entire medical center area’s drug formulary utilization management and CMS/HIDES/NCQA quality metrics which referenced medication usage. Influenced about 250 prescribers’ decision making based on evidence-based medicine and Kaiser/Medicare formulary. Strategically developed plans to achieve goals. Met all set goals in an excellent and exceptional fashion annually. Established a team and led the group to success. Ensured optimum use of existing resources by using process improvement, assessing the amount of work at hand regularly and evaluating return on investment (ROI). Ensured evidence-based medicine/formulary prescribing was used by reviewing patients’ electronic charts and available literature. Educated physicians, nurse practitioners, and nurses on formulary medications and current studies. Conducted prior authorization and patients’ cases for review in various Kaiser Permanente Medical Centers in Southern California. Managed and monitored the adverse events of chemotherapy medications. Designed medication treatment for patients and entered it in the electronic medical record.
