Social Work Intern January 2022- Current Job Duties: Coordinating duties associated with providing educational group, training, completion, and processing of Advance Care Directives for all veterans interested in this service. Complete Case Mix Tool Assessments for Veterans that have a debilitating illness and are homebound need house service. Results of Advance Directives performance, aiming to reward initiatives that increase group sessions efficiency.
