Optimized Eye Care's telemedicine service only

provides a prescription for contacts and glasses and is not a comprehensive eye health exam. Our

doctors recommend all patients between 18 and 50

years old receive an eye health exam at least once every 2 years. For patients over 50, we recommend

you receive a comprehensive eye health exam in

the next three months. If you are experiencing

discomfort, pain, irritation, or sudden loss of vision, visit the ER or an eye care professional

immediately.

Understanding My Prescription

OD (Oculus Dexter): Latin for your right eye.

OS (Oculus Sinister): Latin for your left eye.

SPH (Sphere): Power needed to correct your near or farsighted vision in diopters (D). A negative sign indicates that you're nearsighted and a plus sign is used when you're farsighted.

CYL (Cylinder): Power needed to correct your

astigmatism.

AXIS: Describes the orientation of your astigmatism. ADD: Magnification power in your bifocals or reading glasses to make seeing up close easier.

BC (Base Curve): The curvature needed within the

contact lens to match your eye and provide the best fit and comfort. The lower the number, the steeper the curve of your cornea.

DIA (Diameter): The distance across the surface of your lens, from edge to edge.

Valid until stated expiration date.

Disclaimer: Contact lens dispenser shall not adapt, substitute or change the contact lens prescription. You should return to the initial doctor who fit your contacts to ascertain the accuracy and suitability of the contact lenses. The prescribing doctor shall not be responsible for any damage or injury resulting from negligence of third parties to include but not limited to negligence in packaging, manufacturing, improper care regimen or recommendations

(cleaning, disinfection and wetting) or instructions provided by the seller that lead to over-wear of lenses or improper care of lenses which result in damage to lenses or visual system, and that the prescribing doctor shall not be responsible for contact lens damage, eye injury or damage occurring during the time that lenses are provided by another dispenser. Optimized Eye Care's online vision test is not a replacement for a comprehensive eye health examination. Optimized Eye Care eye care professionals use Optimized Eye Care's on-line technology to evaluate a patient's visual acuity and a portion of the ocular health profile, diagnose refractive error, and issue a prescription for corrective eyewear, where clinically appropriate. Their services are limited to patients between the ages of 18 and 55 who are in good health. All tests are conducted and all prescriptions are issued based on the independent clinical judgement of an eye care professional licensed to issue vision prescriptions within each patient's state. Because the services are not a replacement for a comprehensive eye health examination, we encourage everyone to obtain a comprehensive eye health exam at least once every 2 years. Optimized Eye Care prohibits patients from taking their online vision test more than 4 consecutive years without certifying that they have received a comprehensive eye health exam first. Optimized Eye Care's online vision test is an FDA registered device that can only be used to prescribe glasses and contact lens prescriptions by licensed ophthalmologists or optometrists, depending on the laws governing the state of issuance.
