• Evaluate appropriateness of medication orders for patients based on diagnosis, comorbidity, labs, vitals, other medications on profile, then contact physicians when necessary with recommendations and corrections

• Follow protocol developed to treat COVID patients with up to date information, monitoring labs and making sure proper medications are ordered for COVID patients, with proper of duration of treatment, and discussing with physicians when orders deviate from approved protocols by the hospital's pharmacy and therapeutics committee

• Medication Reconciliation: performing admission, transfer and discharge medication reconciliation then contact the prescribing physician and make recommendations for medication substitutions, changing dosing based on patient’s age, condition, other medications on profile, renal functions, etc…

• Monitor where certain IV push and IV infusion orders are to be administered especially where patients are to be monitored

• Ensure restricted medications are ordered by the appropriate specialties, compounded/ dispensed correctly and appropriately with all necessary precautions, and administered on the appropriate units

• Attend Code-Blue calls: compound and facilitate getting any additional medications not in crash carts, assist in confirming doses or making IVs, looking up data for physicians and answer questions regarding medications

• Attend Stroke Codes: respond quickly to stroke codes - as time is brain, evaluate patients and assist in reviewing medication profile and whether the patient is a candidate for TPA, and facilitate expedient compounding and delivering of the TPA to patients

• Manage medication in the narcotic vault and Pyxis machine and coordinate with pharmacy purchaser, supervisor and technicians according to what is needed.

• Checking and verifying the accuracy of medications filled for Pyxis, cartfill, prepackaged medications, crash trays, crash carts, etc…

o revamped workflow in IV room to be more efficient while ensuring accuracy. Supervise technicians and their techniques, competencies, overseeing all testing necessary to keep IV room in compliance with USP 797 guidelines.

o supervise and train others on using DoseEdge, review the builds and execution of mediations going thru it to ensure accurate, efficient and timely compounding and dispensing of IVs and Chemotherapies.

o manage inventory of IV room especially emergency medications and high cost items o supervise cleaning process/ schedule of IV room

o make sure all items in IV room are in date and stored properly

• AIC Pharmacist - among many tasks I perform the following: o revamped ambulatory infusion clinic (AIC) workflow o integral part in epic build especially for IV medications and Chemotherapy used in our AIC o working with epic to look for/ develop reports to maximize efficiency of managing inventory for medications used in our AIC department

o manage inventory of chemotherapeutic agents, monoclonal antibodies, IVIG, etc… which are all high cost items - making sure we have what our patients need and saving cost for both hospital and patients

o always look for ways to improve patient experience and satisfaction in our AIC by educating RNs, technicians and training other pharmacists on AIC therapy plans, workflow and expectations

o assist nurses and provide resources to help answer their questions but also equip them with tools to use for future questions as well.

o coordinate patient care with physicians and AIC for proper ordering of medications and labs necessary for monitoring to ensure patient safety. o complete review of appropriateness of Chemotherapy orders for patients based on the diagnosis and protocol to be followed, and review proper order entry of Chemotherapeutic agents in terms of proper dose, route, infusion times, priming of the appropriate line, whether filtration is required, whether protection from light is required, and overall final product delivery to ensure accuracy and safety.

o review patient chart and make sure AIC orders are ordered correctly, proper labs are available and within normal limits, physicians notes are available in epic to ensure MD is monitoring the patient and approving continuation of therapy and contacting physician if mentioned notes are not available to make sure MD is aware of any updates to patient’s condition based on chart.

o manage, along with AIC, the medications that are sent to the pharmacy as a “free drug” or a

“direct ship”, maintain the records for those patients, update their therapy plans to make sure the medication is verified and scanned properly to avoid charging the patient since it would be considered “Patient Own Supply”.

o monitoring EUA medications for COVID patients who have mild-moderate symptoms who are eligible to be treated in an outpatient setting to try to avoid their disease progression and hospital admission.
