All payments in the above listed plan are due by 5:00pm. The payments listed above are in addition to your regular water service bills, which are due and payable on the date printed on each bill. There can only be one payment plan in effect for an account holder at any given time. There will not be any additional invoices sent for the payment plan listed above. It is your responsibility to comply with the above listed payment plan and meet all deadlines. If you are delinquent with any payment listed in the schedule above, your account will be eligible for inactivation and services to be disconnected. delinquent during the payment plan period, this will void your payment plan and the full balance on the account will be due. If services are disconnected for non-payment, all applicable charges, including penalty charges, will be due and payable before water service will be restored. Payments may be made over the phone using a check or credit card XVLQJ WKH 'LVWULFW V DXWRPDWHG system at 951-***-****; online with a credit card at www.bcvwd.org; or at the District office at 560 Magnolia Avenue in Beaumont. Payments made at the District office can be made via mail, drop box

(available 24-hours) or in person. Office hours are Monday through Thursday from 8:00 a.m. to 5:00 p.m.. Please be aware that all credit card payments will be subject to a convenience fee. One copy of this notice is for your records.

Please sign and return the second copy of this notice to Customer Service by 12/22/2021 to acknowledge the terms of the payment plan. The payment plan will not be put into effect until a signed notice is received by the District. Failure to submit the signed agreement by the return date will void the agreement.

If mailed, please remember that the District is not responsible for lost or damaged correspondences. Please do not fax your request.
