Change, bathe and put patient into her bed, provide nighttime medication and nighttime meal. Patient was nonverbal and was completely retracted in her extremities. Services for patients ended for the husband decided to put her in a nursing home in July 2013 for she required more in-depth care that was not able to be provided in a home setting.
