RN Case Management is a practice-based RN position that directly supports CCSSLC high risk patients living with mental and physical challenges and dual diagnosed patients. The RN Case Manager

(RNCM) is responsible for conducting comprehensive assessments of patient’s physical, mental, and psychological needs. The RNCM develops care plans to improve outcomes, decrease risk status and minimize infirmary admissions and hospitalizations. The RNCM organizes, coordinates, and collaborates with the primary care physician, interdisciplinary team and with other disciplines in the patient’s plan of care. I have over thirty years of caring and treating individuals living with mental and physical challenges as a DSP, HST and RNCM.

RN (Anchor Habilitation) is an overall nursing practice position that over sees approximately 58 patients serving and caring ICF/HCS individuals living with diagnosis of mental and physical challenges. The RN is responsible for overall direction of the patient care services of Anchor Habilitation Services. The RN plans, develops, implements, coordinates, communicates, delegates, and trains on policies and procedures to ensure the delivery of quality patient care is being provided for all patients served. The RN reviews charts, review consults, review PCP orders, reviews labs, diagnostic test, chart audits, provides training in medication

administration/delegation, in-servicing in infection control/COVID- 19, schedule appointments, physical assessments, CNA’s, staffing’s, meeting ICF/HCS policy and procedure requirements, implementing POC’s and participates in an Interdisciplinary team (IDT). I have thirty years of experience and continue to pursue my education and knowledge base for individuals living with these challenges. Education
