PROFESSIONAL SUMMARY

Primary nurse role (on-call):

• Triage phone calls and assess the level of acuity based on patient’s symptoms and information received from caller.

• Delegate patient visits to other visit nurses based on patient’s needs and acuity. Visit patients and assess their symptoms.

• Manage patient’s pain levels and manage their symptoms (including any psychosocial issues, spiritual concerns, and family dynamics).

• Pronouncement of patient’s death and arranging for funeral home pick up of patient’s body.

• Medication delivery to patients with education performed on the medication. Admission Nurse:

• Assess patients qualifying factors to be admitted to hospice

• Reconcile patient’s medications

• Set-up patient’s initial plan of care (RN frequencies, HOSA visits, chaplain visits and social workers) based on patient’s needs.

• Assess the family needs for bereavement, educate on the hospice philosophy and the death and dying process.

Back up nurse role (Ambercare) (on-call):

• To assist and support the primary nurse with all visits and submit all reports on patients visits with the patient’s concerns and the outcomes to Patient Case Managers.

• Management of symptoms to promote comfort to the patient.

• Education to the family and patients on the patient’s disease process and what to expect during the dying process.

Case manager:

• Management of the patient’s disease process, while promoting safety and comfort to the patient and their loved ones. Development of patient’s plan of care and reconstructing the patient’s plan of care as the patient’s disease process progresses.

• Perform wound care as needed.

• Assess for risk/safety concerns and order DME to promote a safe living environment. CREDENTIALS
