-Paramedic field Preceptor- As a preceptor I work with paramedic students while in their field
hours. Community Paramedic AMR Ambulance - Portland, OR

As a Community Paramedic I worked with Hospital Social workers, Rn’s, and Doctors to provide follow up care for patients following discharge that are at "risk" for readmission. In this position I worked closely with Case Managers and discharge planners as well as social work (TC911) and other various social work agencies (county and state crisis and general DHS case managers) to provide follow up medical care included within my scope of practice as a Paramedic for chronic and acute medical illnesses. I assessed the patients, worked with their primary care doctors, clinical social workers, mental health professionals as well as food banks (animal and human), rehabilitation center, sobering locations, local clinics, Medicare/Medicare, and Veteran’s affairs to establish and help advocate for vulnerable population and create and provide a case plan as well as any current need including emergency food. My patients including but where not limited to elderly, persons suffering from homelessness, drug/ alcohol dependency, mentally and physically disabled, persons living below the poverty level, domestic violence victims. While in this position I excelled in my human relations skills, and adaptivity due to my life experience as well as my vast experience in the field as a Lead paramedic FTO.
