As a Community Paramedic I worked with Hospital Social workers, Rn’s, and Doctors to provide follow up care for patients following discharge that are at "risk" for readmission. In this position I worked closely with Case Managers and discharge planners as well as social work (TC911) and other various social work agencies (county and state crisis and general DHS case managers) to provide follow up medical care included within my scope of practice as a Paramedic for chronic and acute medical illnesses.
