I currently work as a RN Case Manager for 6-12 patients in an Inpatient Hospice Unit. I provide symptom management care for inpatient patients. I perform assessments, administer medications, provide oral report to rounding physicians and update them on the patients conditions, enter orders provided, and whatever else is needed. I am responsible for admissions, discharges, deaths and education. I also perform GIP (General Inpatient) chart audits where I go over patient charts, read the documentation to see if it can support billing for higher level of care. I participate in weekly IDT
(Interdisciplinary Team) meetings where I discuss patient condition, discharge plans, update their personal plan of cares and enter information discussed with all members of the team.
