Job Duties:
Provide assessment, teaching, assistance and evaluation of residents in daily living skills and socialization skills to enhance independence, community integration and overall rehabilitation goals. Utilizing a variety of teaching methods, for example: assisting, role modeling, hands-on, verbal prompts, monitoring, etc. based on individual level of functioning.
Planning and implementing a diversity of activities within the residence, as well as, out in the community based on resident needs, interests, preferences, choices and level of functioning. Perform service coordinating activities assigned for residents, including writing service plans, progress notes, assigned case management duties and maintains all documentation up to date. Maintain the physical property of the residential program to enhance the health and safety of residents and staff.
Respond to crisis and unusual situations using good judgment in accord with program policies. Utilizing first aid and emergency services appropriately, if warranted. Coordinate intervention efforts with staff and outside providers. Informs administration within one hour of incident. Clear, concise and accurate completion of incident report.
