Job Description:

1. Carries out medication orders and facilitates the medication administration process.

2. Makes patient profile sheet for new admissions or trans-out patients, noting the patient’s allergy status and whether the patient is on a charge or cash basis.

3. Screens the medication order for correctness and completeness, ensuring correct transcription to the medication administration record (MAR) by the Charge nurse has been done.

4. Clarifies any medication order found to be a duplication, erroneous, eligible, or ambiguous to the charge nurse before referring to the prescribing physician.

5. Coordinates with the in-patient pharmacy in the procurement and preparation of the medications for patients on charge basis.

6. Collaborates with the staff nurse regarding the procurement of the medications for patients on cash basis.

7. Collects patient’s stocks or maintenance drugs from the patient indicated for the next 24 hours and stores them in the unit compounding area.

8. Checks the authenticity, expiration dates and condition of medications purchased outside the hospital pharmacy before they are accepted and stored in the unit compounding area.

9. Provides accurate labels for all stored medications particularly medicine bottles (suspension/syrups) with the date and time it was opened indicated on the said label.

10. Ascertains all medications for the administration are deposited and stored securely in the station compounding area.

11. Makes inventory of all medications deposited in the unit compounding area every shift.

12. Prepares all medications [e.g. oral drugs, suppositories, drugs by local application (ointments or creams), drugs in nebule form, nasal/ear/eye drops, insulin and intravenous medications], with the exception of controlled/regulated intravenous drugs and high volume parenteral medications, in the station compounding area as close as possible to the time they are scheduled to be given and stores them safely until administered.

13. Dispenses any medicine to the staff nurse assigned to administer the medication while observing the ‘five rights’ of medication administration, ensuring that it is the right drug with the right dose for the right patient through the right route at the right time.

14. Documents in the patient profile the drugs administered on time and notes reasons for any discrepancy in the medication administration process which include but not limited to administration delay, patient refusal, NPO status, missed dose and error of commission.

15. Carries out orders for take home medications; returns unused and discontinued medications to either the hospital pharmacy if on charge basis or to the patient if on cash basis; and endorses to the patient excess medications prescribed for take home use.

16. Facilitates with the unit staff nurse the procurement of take home medications at the request of the patient or the physician, provided such medicines are charged to the patient’s hospital account or with approval from the HMO/insurance through the billing section personnel.

17. Signs the patient’s discharge process slip once all medications have been duly returned; further, signs the discharge clearance slip once patient is about to be discharged.

18. Coordinates with the clinical teacher for nursing students assigned to give medications other than intravenous injections.

19. Offers drug information to the doctors and fellow nurses.

20. Does chart review before the shift ends.

21. Monitors the Bioref temperature at least once every shift and maintains the cleanliness of the unit medication compounding area.

22. Follows up with the charge nurse in ensuring telephone orders and verbal orders are transcribed by the prescribing physician within 24 hours from the time the order was made.

23. Posts surcharge fees to medications purchased by patients outside the hospital pharmacy following hospital guidelines.
