My name is John Blake. I have over thirty years in the medical field. The last twenty two
years as a Physician Assistant. I have emergency room experience, as well as, clinical
experience in Family medicine. I have received awards from youth programs I donated
time to in the community. I am a medically retired Veteran from the United States Army,
and also a United States Navy Veteran. Father of four children, and grandfather of four
grandchildren. Im honest, loyal and experience.

PHYSICIAN ASSISTANT, WEXFORD HEALTH, HUTTONSVILLE,WV, 2017-PRESENT
Performed patient evaluations in emergency conditions and chronic conditions. Carried
my own case load of patients and assisted my supervising physician in every day task in a
clinical aspect of medicine. Performed history and physicals on new patient. Developed
specifics care plans for patients. Evaluated patients for the need of referral to specialized
care. Facilitated in transfers and acceptance of patients with need for specialized care at
other facilities.

Worked as a Physician Assistant sixty plus hours a week. Dividing my time between the
Emergency Room and the Rural Health Clinic, hospitalized based. Spent most of me work
experience with complete autonomy, and physician coverage by telephone. Carried my
own case load for chronic conditions in the rural clinic. Evaluating, and treating patients
for chronic conditions, to include referring patients for specialized medical care. Evaluated,
treated and stabilized patients for transfer in the Emergency room. Facilitated the transfer
and acceptance, for those patients needing further care by a higher care facility.

1986-1992
I served in duel positions in the US Army. My primary position was the non-commissioned ocer
(NCOIC) of the Troop Medical Clinic thirteen. In this position I was in charge of the personnel
that worked at the clinic. I made job assignments and assessed individuals annual evaluation of
job performance. Made recommendations for promotions and assisted in the selection of those
individuals. I performed the triage of sick call individuals, and selected the patients that needed
seen by the clinic physician. I ordered medical supplies, as-well-as administrative supplies. My
second role was the Platoon Sargent of first platoon, Seventh Special Forces Group. In this role I
supervised and managed seventy five to one hundred soldiers in the daily routine required by
military standards. This included the scheduling of work assignments, disciplinary actions and
selection of soldiers deserving of promotional awards facilitated and scheduling leave and
assigning coverage of these positions. I was in the first combat action Desert Storm, Operation
Iraqi Freedom. I used my military skills, and my medical skills to protect the UnitedStates and
her soldiers. I received a Meritorious Service Medical and a Purple Heart Medal.

Worked as support for the US Navy SEAL Team Two. Providing medical care while in a non-
deployed status. Served as a NCOIC of the physical therapy department at the Troop
Hospital. Providing assistance to the Physical therapist on duty. I also was there to do the
Histories and initial exams on new patients. Assisted in the application and removal of
orthopedic braces and cast. During my term of service, I received several leadership and
accomplishment. Awards. Highest being the Presidential Service Medal.
