PROFESSIONAL SUMMARY:

NURSE PRACTITIONER:

Collaborate with physicians to deliver health care services to skilled nursing facility residents as approved by Collaborating Physician in person or by telecommunications.

Perform patient assessment and diagnosis, ordering diagnostic and therapeutic tests and procedures, interpreting and using the results of diagnostic and therapeutic tests and procedures ordered by APN or another health care professional.

Ordering treatments, ordering or applying appropriate medical devices, using nursing, medical, therapeutic and corrective measures to treat illness and improve health status in providing services as authorized by the Collaborating Physician.

Prescribing medications and other Schedule II, III, IV and V controlled substances, as defined in the Prescription Drug Act and as allowed under the Nurse Practice Act and the rules of APN’s profession.

Perform screening and assessment of chronic pain in an outpatient setting.

Obtain history and physical examination, medical diagnoses, manage chronic pain and decrease opioid use within the chronic pain environment.

Evaluate patients’ response to treatment and collaborates with the medical director/Supervising physician for any adjustment in pain medications and other interventions to improve patients’ quality of life.

Assess pain, monitor pain levels and side effects of pain medications, consulting and advocating for staff and patients, leading and educating staff related to pain management.

Perform injections under US guidance.

ADMINISTRATOR:

Implement an effective budgeting and accounting system, assures accuracy for billing procedures

Assess employees on an ongoing basis to ascertain their understanding of policies and procedures

Discipline employees as necessary

Use statistical data to determine quality and quantity of services

Maintain compliance with applicable federal, state, The Joint Commission and local rules and regulations

Develop, implement and evaluate budget plan and cost control policy and procedures

Recruit employees and retain qualified personnel to maintain appropriate staffing levels

Direct staff in performance of their duties including admission, discharge and provision of services to patients

AGENCY SUPERVISOR:

Plan, direct, coordinate and monitor the delivery of all direct and indirect services to Agency Home Care Clients

Assist in the planning and development of Agency Structure, clinical and administrative policies and procedures and the annual operating and capital budgets

Assist supervisor in managing clinical teams and planning

Evaluate the performance of Clinical Supervisors and assists them in developing skills and techniques to evaluate performance of staff that report to them

Assure clinical records are maintained in compliance with state and federal laws including Medicare, ACHC Standards and conditions of participation

Conduct overall employee orientation and competency assessments

Identify systems to recognize client needs, respond to client needs and to measure outcomes if Agency Interventions

CASE MANAGEMENT

Perform comprehensive assessments of client status including physical, psychosocial, environmental parameters and assess client needs for other disciplines

Complete comprehensive assessments including OASIS data set within 48 hours of referral.

Provide skilled interventions with a focus on achieving realistic client outcomes within a specified time period.

Initiate actions to improve the quality of patient care. Take appropriate actions independently and responsively in home care situations. Initiate referrals as needed to other health team members as delegated by agency supervisor.

Participate in staff in service programs, conferences, workshops and meetings

Responsible for care given by HH Aides and LPN’s, Supervises Home Health Aides assigned every 14 days and as needed. Supervises Licensed Practical Nurse assigned once every 30 days and as needed.

QUALITY ASSURANCE

Review and analyze data and documentation, maintaining compliance applicable with Federal and State regulation requirements.

Identify problems deficiencies in the clients chart and develop a plan for its resolution.

Audit reviews OASIS completeness, documentation of skilled services provided care plan update medication update telephone orders and other related documentation/supplement note necessary.

Assist in receiving patient/client referrals for possible services to be provided and refers client to community organization as appropriate.

Coordinate with MD and provides summaries of patient’s progress and status

Responsible for taking referrals from referral services and initiates, assign/notify SOC nurse

CODING CONSULTANT

Perform clinical validation audits and interpretation of medical documentation to ensure capture of all relevant coding based on CMS coding manual conditions that are applicable to Medicare Risk Adjustment Reimbursement initiatives or HHRG

Perform root cause analysis and submit recommendation for appropriate change management when clinical validation audits and interpretation of medical documentation indicates lack of member access and provider involvement to manage chronic risk conditions.

Analyze data from various sources including but not limited to inpatient hospitalizations, diagnostic testing, outpatient procedures and services, home health care services, durable medical equipment, rehabilitative therapies, pharmacy reviews for possibility of chronic condition codes missed.

Upon completion of medical record validation audit, compiles detailed findings and relevant supporting documentation for inclusion in Risk Adjustment submittals.

Prepare written reports for Medicare Reimbursement including supportive documentation.

Advise manager of possible trends in inappropriate utilization (under and/or over), and quality of care issues.
