My recent role as hospice administrator for Comfort hospice has resulted in a positive productive environment. The office now has had a dramatic improvement in CHAP HIS and CAHPS scores.The office has shown growth and staff development along with training of additional staff. As Regional Director of a multi-state hospice company, developed two hospice care facilities.

CHAP certification achieved under my direction for both facilities, one of which received zero deficiencies

All four facilities under my supervision attained greater than 100% net income to budget. Performance improvement including QAPI and ADR processes My case manager duties included the following:

Documentation for case conferences Confer with physicians to create Appropriate plans of care. Counsel and educate family on long term plans. Health Assessment Data evaluation and exceptional documentation to Determine appropriate nursing diagnosis.

Ensure compliance with government guidelines.

Support and perform case management.

Provider relations with hospitals and physician providers. My long term care experience included oversight of the entire house of 176 bed facility included hiring, training, and recruitment of staff. During my supervision I also provided hands on care of patients and was strong in Medicare documentation Nursing experience with a variety of patients and payor sources.
