Objective: To work in a healthcare environment to achieve optimum outcomes for the patients, their families and the healthcare community I serve.

June 2020- Aug. 31, 2020-Core medical Travel assignment as Hospice case manager with Petaluma Hospice.

August 2019-May 2020: Heartland hospice- Santa Rosa CA. RN case manager serving a diverse group of patients in a variety of settings: home, Rcfe’s and skilled nursing facilities. Managing a multitude of patients at the end of their lives. Helping patients and their families to understand the patient’s disease processes; helping patients to be as comfortable as possible and to ease symptoms associated with their disease process as much as possible by collaborating with the hospice medical director, attending Md and the patient’s IDG team to provide the best care possible for patients and their families.

January 2018-August 2019-Collabria hospice- (formerly Napa Valley hospice)-RN case manager, admissions nurse, hospital liaison for QVMC. I worked a multitude of positions, doing whatever was needed to help the hospice community I served.

September 2017-January 2018- Continuum Hospice. RN Case manager in Napa- working closely with IDG team, patient’s attending Md, patients and their families in whatever setting the patient considered to be their home.

November 2015-September 2017- Collabria hospice- Napa CA. – RNCM and hospital liaison. Worked in my community of Napa that I that has been my home for 40+ years. Caring for patients as they face their terminal illnesses, providing a calm presence and knowledge to help patients and their families to deal with the emotional, physical and spiritual challenges they are facing. Working closely with IDG team to achieve the best outcomes for the patient and their family. In my role of case manager, most of my patients were at Veterans home in Yountville. I had patients in Memory care, the skilled nursing facilities, independent living and hospital setting, which is a population that I thoroughly enjoyed. I formed many good relationships with the nurses, NP’s, MDs and LCSW’s that I cared for.

** Gap in employment 11/14- 11/15- I moved out of state, back to Michigan where I was born to assist in the care of my terminally ill sister in law.

April 2014- October 2014- Heartland hospice Santa Rosa CA. RNCM -RN case manager serving a diverse group of patients in a variety of settings: home, Rcfe’s and skilled nursing facilities. October 2013-April 2014 Hospice by the Bay, Sonoma CA. Hospice resource RN. As a resource nurse I saw patients for routine visit for their RNCM, as well as any other patient care issues that might have come up on a prn, often “urgent basis”. The focus being pain and symptom mgt; with the goal to always keep patient as comfortable as possible.

October 2012-October 2013- Napa Valley hospice and Adult day services: Palliative Care Nurse Navigator for the Transitions Palliative Care program. My nursing focus was working with patients and their families in the local skilled nursing facilities, RCFE’s and Veterans home in Yountville. The primary goals of care ; advance care planning, educating patients and their families on patient’s life limiting condition and goals of care. Working with patient and patient’s attending Md on patient’s medication regimen to optimize best outcome for patient’s symptoms.

August 2007- March 2012- Sutter VNA (now Sutter care at home)-Home care RNCM for a variety of complex patients with varying acute and chronic conditions. I was also a case manager for the AIM program (Advanced Illness Management) which is Sutter’s outpatient Palliative care program. The goal of care for these patients was to assist them with tools and guidance to minimize pain and symptom management. Working with tha population beginning in 2008 required specialized training and competencies as well as collaborating often with other Aim RNs, MSW, community liaison and patient’s attending physician. The Aim program served as an adjunct between home care and hospice. Many Aim patients transitioned from the Aim program to hospice when appropriate and the advanced care planning and identifying patient and their family’s goals of care was an invaluable tool for them.

In addition to being a home care and Aim nurse I also worked as the community liaison at Sutter Solano in Vallejo and as an Intake nurse for the busy homecare/Aim programs which routinely had a census of 500+ patients. I am very knowledgeable of Medicare, Jhaco and NHCPO guidelines. I have a deep broad knowledge of community resources for patients and their families in both Napa and Solano counties.
