Provides care management services for the Acquired Brain Injury Waiver and The
Connecticut Home Care Program for Elders, that arranges for non-medical home care
services as part of an overall care plan to maintain adults with physical and/or mental
disabilities, age 18-64 inclusive, in the community (ABI Waiver), as well as for adults age 65
and older inclusive, in the community (CHCPE).
