Office Assistant-Jamestown Psychiatric/Advance Medical Practices: Part-time office assistant with duties that include data entry of critical records, patient scheduling, receiving payments, taking vitals and greeting patients as they enter for their appointments. October 2008 to Current: Social Worker--Lutheran Social Services--Jamestown, NY I am currently the social worker assigned to the 29 bed long term care unit and Warner Place, the adult daycare facility on the Lutheran Social Services campus. Responsibilities include completing admission paperwork, documenting advance directives, facilitating room changes, assisting in developing health care plans, investigating and reviewing incidents and accidents, and communicating with family members regarding the quality of life and health care needs of those residents assigned to me.

Additional duties to those stated above included administration of the budget for the social work department, attending QAPI and DSW meetings held throughout NY, making referrals to OPTUM, supervising other social workers and running resident council meetings. February 2003 to October 2008 Social Worker--Chautauqua County Hospice I assisted patients and their families in dealing with end-of-life issues ranging from the emotional to the financial. Based out of the then Mayville central office, this involved extensive travel throughout the county to meet with those receiving Hospice care in their homes and often with their loved ones and caregivers.

March 2000-October 2002 Youth Outreach Worker, Private Industry Council-Dunkirk-Jamestown I worked with recent high school dropouts in an attempt to place them into GED, life skills or job training programs. In this position I networked with administrators and counselors from school districts throughout Chautauqua County and also with the agencies that comprised the Youth Outreach Network which was the provider of many of the services which were available to my cases.
