- Work Title: Charge LVN - oversees the station with 30 residents with 3-4 assigned CNA each shift. Bedside care assessments at the beginning of each shift and continue care upon report. Passed daily medications per eMar including V/S daily checks. Make sure that residents are prepared and ready for appointments such as dialysis and doctors scheduled appointments, cohorts with social services scheduling and transportation.

- Started change in condition/SBAR if there’s change per resident’s baseline. Called Primary Care Physician and reported current observation. Carried out orders upon doctors orders. Followed up laboratory results and reported to the doctor and received orders as necessary. Sent resident to emergency care for further evaluation as indicated.

- Make sure all the residents are turned and repositioned Q2hours and prn. Ensured that residents are out of bed and attended activity services, and ADL’s are attended each shift.

- Cared for Long Term care residents, residents with trach and vent, resident’s with G-Tube, resident’s with wound, foley cath, wound vacuum.

- Passed daily medications by mouth, including B/P, diabetic, cholesterol, OTC, psych, controlled meds, topical, eye, ear drops, transdermal, IM, & hypodermal injections

- Administered PPD, heparin, lovenox, glucagon, insulin, procrit, and PNA & flu vaccine as indicated. Mechanical Ventilator residents nursing intervention cohort with RTs. Done admissions with head to toe assessments & paperworks associated with admissions per facility’s protocol. Patient Care / Beside nursing skills and interventions

- Done nursing management on residents with Vent and Trach, neurological check monitoring, Peg-tube, G-Tube with feeding tube and pump, Foley catheterization, straight cath, compression boots, pulse oximeter probe and Accu-Check with its nursing intervention.

- Assessment and reassessment, head to toe, carried out Change in Condition, SBAR, incident report, notify MD, accomplished short and long term care plan goals, and carried out MD’s orders asap

- Start and discontinue IV’s, admission and discharge intervention, & associated documentations.

- Foley catheter insertion and straight catheterization for collecting specimen using sterile technique

- Oxygen managements - start and D/C with pulse oximetry monitoring

- Cared for Patton clients; medications & IV passes, bedside assessments ( related to mental health) Other nursing skills and Treatment Administration / Wound Care

- All stages, post-surgical wound, excoriation, abrasion, skin tears. Proficient in using the ffg tx meds: Santyl, Silvadene, Calcium Alginate and CaAl Ag, sure prep, proderm, Unna boot, hydrogenate, hydrocolloid dressing, kerlix, gauze, medfix, and topical A&D, Zinc oxide, NS, TAO, Steri strip, dry dressing, etc and used negative pressure wound tx management.
