WORK EXPERIENCE
02/18-Current: Contracted Medical Provider for Barton Associates. Per Diem Telemedicine NP, reviewing and approving applications for DME such as back and neck braces, ankle and wrist braces, and analgesic creams for treatment of muscle and joint pain. Approval of patient applications takes place via cloud-based encrypted internet sharing platform.
02/18-09/18: Jackson Coker Associates/Colorado Department of Corrections. Contracted Medical Provider at the Delta Correctional Facility in Delta, Colorado, providing comprehensive medical care to incarcerated individuals for up to 24 hours per week. Medical services include emergency care, health and wellness exams, treatment for acute injuries and illnesses, wellness visits, and providing chronic care treatment for both physical and emotional conditions. This is a contracted position that will end when the Colorado Department of Corrections hires a permanent full-time Nurse Practitioner.
04/16-02/18: Correct Care Solutions Part time mid-level medical provider, providing health care for acute and chronic conditions at the Mesa County Detention Center in Grand Junction, Colorado, and Montrose County Detention Center in Montrose, Colorado. Following established, quality based protocols, the medical provider performs medical exams for both acute and chronic conditions. This requires a broad knowledge of both physical and psychological conditions, the ability to diagnose and treat individuals who may have a challenging personal and criminal history, the possession of social skills that enable the establishment of good working relationship with both jail and medical staff, and the ability to establish a sense of trust with individuals who are incarcerated within the criminal justice system.
04/16-01/17 Medical Provider, Interim, for Foresight Family Physicians, Grand Junction, Colorado. Worked as a temporary part-time Family Nurse Practitioner with 4 other providers, while Foresight Family Physicians searched for a full-time mid-level provider. In December 2016 a full time Family Nurse Practitioner was hired; I remain available for future fill-in as needed. 01/15-06/15 Medical Provider for the Paiute Indian Tribe of Utah (PITU). As a family nurse practitioner, I provided medical care, preventive screenings, family planning and many other services to the members of the Paiute Indian Tribe of Utah, and to the members of the surrounding community. The clinic is located in Kanosh Utah, on the Kanosh Reservation of the Kanosh Paiute Band. It is one of four clinics in Utah that are under th supervision and support of the (PITU). The headquarters for the PITU agency is located in Cedar City, Utah.

Conducting in-home examinations at the level of a licensed, trained Advance Practice Registered Nurse.

Complete an “Ask Your Doctor” worksheet.
Leave the worksheet, a contact sheet. Return the signed examination form or eHRA (electronic Health Risk Assessment) to Center.

Successfully wrote and applied for Federally Qualified Health Center Look-Alike Status through HRSA. Successfully applied for and achieved Health Center Shortage Area in Midvale, Utah, through HRSA. Maintained Worker’s Compensation Fund status, payroll, taxes, local, state and federal licenses and permits.
Developed health outreach programs that serve the clinic clientele, planned and conducted Board of Directors meetings, work closely with community partners, develop volunteer programs, supervise and mentor medical residents, nurse practitioner students and registered nurse students. Organized annual Midvale Health Fair Community Clinic from 2003 through 2010.
