Gastroenterology Nurse Practitioner (June 2017-July 2020)

Obtains patient history and physicals in an outpatient clinical setting with particular focus on Gastroenterology to include acute/chronic abdominal pain, upper/lower bleeding in the digestive tract, GI malignancies (colorectal cancer, gastric cancer, and pancreatic cancer), diverticular disease and other diseases of the colon (IBS, Ulcerative Colitis, and Crohn's disease), gallbladder disease, GERD, hemorrhoids, malabsorption disorders (celiac disease and lactose intolerance), nausea, vomiting, ulcers, and unintentional weight loss.

Engages in multidisciplinary approaches when composing plans of care for the Veterans and acts as a patient advocate with other disciplines including general surgery, mental health, primary care, and social workers.

Coordinates care and ensures continuity follow-up for Veterans with medically complex GI history. Encourages the utilization of innovative treatment modalities for Veterans requiring more frequent follow up by utilizing RN face to face or video on demand visits. This has included coordinator of care for patients requiring temporary out of state GI care with successful IV infusion for multiple biological agents.

Serves as a consultant advising referring providers from various disciplines across DVAMC and surrounding CBOC’s who enter consults into CPRS. After each case is reviewed, it will further determine which GI clinic is most appropriate to meet the Veteran’s healthcare needs. If the Veteran needs additional consults for other disciplines ( i.e. general surgery, interventional radiology, and hepatology), this will then be advised to the requesting provider. Managing and tracking multiple consult lists in this role within the GI service.

Participates in multi-disciplinary conference that includes GI, General Surgery, Colorectal surgery, Pathology, Radiology, Radiology Oncology, and Hematology Oncology. Has presented GI patients to committee for further review of imaging, procedural report, and pathology, then in this setting completed patient notes documenting recommendations. Follow up includes collaboration with surgical services Advanced Practice Provider and General Surgery fellow for procedure follow up and placing consults.

Educates and works closely to mentor four GI RN’s in regard to the management of colonoscopy prep orders, assessing driver situations for procedures (including determining whom is appropriate for inpatient lodging prior to and/or after procedures), assessing patient’s current medical status, and how it may affect their upcoming EGD/colonoscopy procedure. Encourages the GI RN’s to use social work consults for pre/post procedure housing recommendation while also identifying resources in community

Participates in modifying clinical practice by providing recommendations and implementing changes at a service level to reduce the average wait time for the outpatient GI General clinic and GI Fellows continuity clinic.
