The Urdaneta Doctors Hospital with a 50-bed capacity was established in 1975. Four years later,the management invited Mayor Amadeo R. Perez Jr. to join the Board. The hospital was renamed URDANETA SACRED HEART HOSPITAL,INC(USHHI).

Mayor Perez provided strategic direction guidance to management. He wanted the facility modernized to cater to the eastern part of Pangasinan.He initiated a total facelift,expanded medical services and created new management positions. In 2003,when he became the Chairman of the Board,he immediately embraked on new technologies in medicine and surgery.He installed Cristino L. NaguiatJr.,the Chief Executive Officer who transformed USHHI into a haven for healing,recovery and wellness comparable to best in Metro Manila.

In 2006,the USHHI after having complied with the policies laid by the Department of Health(DOH) and the Philippine Health Insurance Corporation(Philhealth) adopted the new Hospital Information Management System(HIMS),it was accredited Tertiary Level and became the recipient of the prestigious Center for Quality Care award,a distinction and honor it has maintained until today.

In October 2012,DOH revised the “Rules and Regulation Governing the New Classification of Hospitals and Other Health Facilities in the Philippines” and reclassified hospitals and other facilities beased on the new standards. In 2013,USHHI was reclassified to level 2,a new designation level for the old level 3.

With and increasingly aware growing population on the latest developments on healthcare,the USHHI Board decided to construct a new building to house its latest acquisition of state-of-the-art hospital equipment complemented by improved convalescent rooms and conductive for working spaces for its medical personnel and staff.

Today,the USHHI has in its roster medical specialist and support staff capable to attend all kinds of medical complaints. The soon-to-be completed medical arts building is a testimonyof its social commitment to quality dedicated service to the medically impaired. Job Description

Independent Nursing Function

a.Obtains information or data about the condition of the patient such as vital signs,history taking and physical examination.

b.Monitors vital signs, measures intake and output.

c.Accurately document the procedures treatment and nursing care rendered including patient’s condition upon endorsement.

d.Receives and endorses patients,unit,equipments and articles.

e.Identifies nursing needs of patients.

f.Plans,gives and evaluates nursing care to patients.

g.Communicates to the physician about the patient’s concerns and needs.

h.Promotes safety,mobility and hygiene,and gives comfort to the patient.

i.Assists patients in moving,maintaining proper alignment of patient’s body and utilizing good body mechanics.

j.Takes and records vital signs,fetal heart tone and weight of the patient.

k.Reports untoward signs and symptoms to physician and document it accurately.

l.Observes medical asepsis such as:

Proper handwashing technique

Terminal disinfection

Isolation of patients

Proper gloving and wearing of mask

m.Observes signs and symptoms,institutes remedial action when appropriate and record theses in patient’s chart.

n.Assist physicians and provide screen for privacy when examining patients and during emergency treatment and procedures.

o.Check intravenous fluids,urinaycatheter,oxygen inhalation,nasogastric tube(NGT) and other procedures for patency.

p.Documents all procedures before and after the performance of the procedure.

q.Collects specimen such as urine,stool,sputum and bring specimen to the laboratory for examination.

r.Assist in the preparation of patients who will undergo procedures such as X-RAY,ULTRASOUND,CT-SCAN,ARTERIAL AND VENOUS SCANS,EGD,2D ECHO,PULMONARY FUNCTION TEST and other tests.

s.Ensures privacy of patients at all times and interprets the patient’s rights.

t.Safeguards patients confidentiality

u.Prepares patients for emergency operations and ensures that all required task on thghe operative checklist are carried out.

v.Do proper endorsement regarding patient’s condition and continuity of care.

w.Provides health teaching to patients,watchers,relatives and significant others.

f.Initiates IV therapy and blood transfusion to the patient.

g.Administers and charts prescribed medications and patient’s reactions to the medications.

h.Observes for any signs and symptoms,institutes remedial actions then records in the chart for any untoward reaction.

i.Facilitates discharge of patient and give shealth care instructions and simple nursing procedure in home care.
