WellMed Medical management is a managed care organization (MCO) with contracted physicians throughout San Antonio and surrounding areas. I performed high-risk assessments (HRA) at neutral facilities or at the contracted provider’s medical office. Position required being flexible, adaptability, along with great etiquette and communication skills. Time management and patient interaction was paramount to being invited back into the offices and patient compliance with annual health screenings. My ability to show empathy, professionalism, teaching skills not only benefited patients and their providers, it benefited the medical assistants, LVNs, RNs and other doctors and staff. The HRA includes a comprehensive health, medical, and surgical history, extensive medication and supplement review along with medication therapy management assessments to identify less than best practice medication prescribing, possible medication interactions and reactions that could possibly endanger the member or cause undesirable outcomes. I performed physical exams evaluated screening and laboratory values to identify patients within the MCO who are diagnosed with high risk diagnoses and used the information obtained from the assessment to provide patient education to improve the members understanding of their health condition(s), utilization and compliance of medications, and keep members up to date on current recommendations for screening tests to improve quality of care. I was required to apply ICD-10 codes and buddy codes relevant to each patient’s particular health status. The intent of the assessment is to assist in reducing unnecessary hospital emergency room visits, improve patient education and compliance which in turn improves the overall health and welfare of the member ultimately resulting in better patient outcomes at a lower cost for the Medicare Advantage Plan.
