INFORMATION FOR APPLICANTS—HOME HEALTH AIDE (HHA)

AND CERTIFIED NURSING ASSISTANT (CNA)

To complete the application process, please provide us with the following documentation:

CNA License or HHA Training Certificate (minimum 40 hours)

Automobile Insurance

Valid Driver’s License

Additional identification to complete INS Form I-9 (see instructions)

CPR Certification Card

Available Continuing Education/Inservice Training Certificates:

Alzheimer’s Disease and Related Dementias Training totaling at least two hours. If you have not had this training, you may provide our agency with proof of training within 9 months of hire.

HIV/AIDS Training Certificate

CNA’s Only: Documentation of training required by the Department of Health to maintain your professional license (such as Infection Control, Domestic Violence, etc.)

Tuberculosis (TB) Screening Results or Chest X-Ray Results

Level 2 Criminal Background Screening: You must have satisfactory results from Level 2 background screening prior to working with our agency. If you have already had Level 2 screening conducted and have not had a break in employment greater than 90 days, please provide the agency with your social security number so it can access your screening results on the State of Florida’s database. If you have not already had the screening, please ask the agency for additional information.
