Supervision and training of Med Technicians and pharmacy program via eMar System. Overseeing clients in Senior Living Communities with medication assistance program. Assessment of the geriatric community for new facility and Private duty clients. Collaborating/Implementing plans of care with family, Physicians, skilled service providers, pharmacy and discharge planners/SW to meet the needs of client returning to senior facilities or their homes.
