Completing field-based visit to identified chronic complex care members in their home community, and doctor’s office for complex care management of chronic disease in Brooklyn and Staten Island. Utilizing clinical critical thinking when completing a comprehensive general assessment, PHQ2/9 behavioral health assessment tool and a home risk assessment and available community resources for support within the home environment.
