Collaborating with staff, and physicians on patients plan of care, and discharge planning, upon

admission. Review admission, complete interqual review, and assess patient’s symptoms and complaints, to consider versus the recommended criteria, and the patient is in the appropriate level of care. Assist with discharge planning, i.e. home health, rehabilitation, transfer to facility of higher level of care, and assist with obtaining durable medical equipment. Worked closely with reauthorizations on medications that were not on insurance formularies. Through timely reviews, improvement of patient status, and timely discharge planning, as a case manager, I was able to participate in the decrease length of stay, therefore, financial saving for the hospital stay.

Immigration Custom Enforcement, El Paso Processing Service Center- 8915 Montana, El Paso, TX 79925. Sept. 17, 2018 to April 7,2019.

Staff Nurse- Providing direct care to detainees, to include detainee intake physical assessments, social assessments, referrals to providers, and treatments as ordered. Treatments included medication administration, IV medication administration, IM medication, NG tube feedings, wound care, respiratory treatments, and drawing daily labs. Worked closely with providers to report any changes in status for ordered treatments, and make appropriate referrals as needed. Completing pre-assessments for detainees that would be leaving facility for outside scheduled appointments and follow up on treatments from outside physicians upon return. Participated in the care of detainees in sick call, where detainees sign up to be assessed for non-emergent conditions such as headaches, cough, colds, and allergies, which can be treated with over the counter medications provided by staff nurse at discretion. Responsible for follow-up with detainees that have symptoms that worsen and require further treatment. Emergent treatment to detainees that may become involved in altercations with other detainees, sudden illness, change in status, requiring emergent stabilization and/or transfer care to higher level of care through Emergency Medical Services.

Fresenius Dialysis Care- 1225 E. Cliff #1-A, El Paso, Tx 79902. Jan. 3, 2018 to Sept. 11, 2018

Peritoneal Dialysis Nurse-Conducting initial assessment and determining best teaching strategy for patients and family members, to perform peritoneal dialysis treatments in the home. Stressing the importance of having treatment in a clean and sterile environment as possible, to prevent infection. Reinforcing the importance of the teaching and understanding of the process and reasoning behind the process. Under the direction of the physician, initiating first flush post operatively, sterile dressing changes, troubleshooting peritoneal catheters, assessments to assure patients do not develop infections, and treating patients when there are indicators of a possible infection. Administering antibiotics into peritoneal cavity as ordered by physicians. Teaching and reinforcing teaching with patients when using cyclers to complete treatments, or when patient is performing manual exchanges. Scheduling and completing monthly lab draw on all patients to assure dialysis treatments are clearing enough waste product. When lab results indicate abnormalities, notify physician so that adjustment can be made to exchange prescription for optimal results.

Pediatric Care Manager—Visit to patient home to conduct initial assessment of patient health history, durable medical equipment for home use, services such as physical therapy, occupational therapy, and speech therapy.
