Family Nurse Practitioner- Home health, telemedicine visits and telephonic case management. I assess members with acute and chronic conditions such as Heart Failure, uncontrolled Diabetes, uncontrolled Hypertension and mental illness. Our role as FNP’s at CHP involves using critical thinking skills to assist members in a home setting. Determine what the member needs in the areas of medical, social and psychiatric health. I am responsible for my team which includes a medical assistant, medical liaison and a behavior health specialist. Provide education on their acute episodic and chronic conditions and order appropriate labs and test. Together we work on achieving optimum health and avoiding unnecessary emergency room visits. At CHP we use a tablet for telemedicine visits and to monitor blood pressure, blood glucose, weight and pulse oximeter.
