HOSPITAL PHARMACIST
Process medication orders and prescription to
provide medications ordered by physicians for the
patients.
Ensures the availability of drugs and medicines for emergency cases in the nurse station, emergency
room, intensive care unit and operation theatre.
Discuss medication order clarifications with the prescriber, document any pharmacy records and
inform others of medication order changes.
Provides drug information to the healthcare professionals and to patients.
Review physician orders for possible therapeutic duplication, contraindication, interactions, allergies and formulary status of drug.
Ensures the proper storage and preservation of drugs.
Checking floor stocks and expiration of drugs and supplies. Monitors and checks drug movement and expiration of assigned area-of-responsibility.
PHARMACIST /IN-PATIENT
Preparing and supplying medicines to the in-patients through the nursing staff for treatment and on drug orders from the Doctors including discharge medicines.
Preparations of iv fluids including High Art medications and compounding of medicines as needed.
Maintenance of stock of drugs in pharmacy and in various ward units and inspection of proper storage and
administration of drugs.
Charging of the ward stock drugs used from the ward units for the in-patients by the nursing staff.
Monthly ward audits in IP, ICU, NICU, OPD, ER
departments and preparations prior to JCI inspection.
Crash-cart medicines are monitored time to time and refilled timely as per the necessity.
Monitor patient medication profile.
Medication reconciliation.
Checking of ambulance medications.
PHARMACIST/OUTPATIENT
Dispensing of medicines with proper labelling
instructions and monitoring drug therapies. Works to legal and ethical guidelines to ensure correct and safe supply of medicines to the patients.
Review physicians original drug order for
appropriateness prior to dispensing.
Discuss medication order for clarification with the prescriber, document any changes, inform others of any order changes and document all drug related
interventions.
keeping the track of drugs expiry dates.
Proper inventory maintenance of the stock of the drugs, its storage and removing the short expiry drugs from the shelves.
Experience with the implementation of JCIA standards in the department.
