Executive Summary:
As Lead Social Service Practitioner, among my duties as an advanced professional social worker, my duties consist of providing diagnosis, assessment interventions, and case management support to individuals, groups, children and families with complex health, physical and economic problems. Some of the duties consist of risk and assessment bio-psychosocial conditions and evaluate clients social functioning as well as family and group dynamics. The formal assessment include but are not limited to appraisal of depression, interaction and risk, behavior, and growth and development and substance abuse. Additionally, the assessments are conducted on-site inspections, visits, to determine physical and emotional welfare to analyze and assess client needs for services. I also provide emergency and statistic report of case records findings and recommendations. I mentor new employees, orient trainees and court, community resources as appropriate. Additionally, I am also a Medical Social Worker per Diem for Community Hospital of San Bernardino. My duties consist of assassinating Case Manager RN review and coordinate after care and long term care services after discharge orders from hospital. Working closely with patient and patient’s family to determine the appropriate care and transition from the hospital back home. Among assessments and care coordination, I also provide care plan needs and developed safe discharge plans for patient to live independently. I collaborate closely with RN Case Managers, MD’s, and Emergency Responders and after care Skilled Nursing Facilities and Long Term Care.
