JOB DESCRIPTION:

As a senior Health Career, our primary purpose is to able to promote optimal human health and function by delivering appropriate nursing care to patient.

1.Monitor the patient’s health by taking vital sign every now and then.

2.Feed patient’s as directed by a qualified nurse

3.To record liquid intake and report dietary intake as well as fluid output.

5.Assist patient to mobilize.

6.Conduct basic education to the whole family about nutrition on therapeutic purpose to avoid obesity, diabetes and other illness.

As a Physical Therapist, a health care professional, our primary purpose is to be able to promote optimal human health and function through the application of scientific principles to prevent, identify, asses, correct, or alleviate acute or prolonged movement dysfunction.

JOB RESPONSIBILITIES:

1.Works in conjunction with the other physical therapist in the provisions of physical therapy services in the facility which include the following:

: Patients evaluation

: Initiation and delivery of treatment procedures such as physical therapy agents, therapeutic exercise, gait training, and other modalities of care upon the directive of the physiatrist.

: Assessment of the patient’s progress and modification of the treatment plan as necessary.

: Accurate and professional documentation.

: Assist the Clinical Supervision in the orientation and training of the interns and volunteers.

2.Work as part of the Health Care Team, Maintaining effective relationships with other health care professionals, clients, relative, and other involved in contributing to the client’s care.

3. Perform other related duties as assigned from time to time.
