• Identify surgical candidates, perform pre-operative work-ups/physical examinations and coordinate care between physicians and the patient.

• Order required pre-operative testing, review lab results and plan actions based on results. Order consults and provide counseling as needed.

• Analyze and interpret patient’s histories, physical findings and diagnostic information in order to develop appropriate diagnosis and treatment plans within the care team.

• Write admission/discharge orders, maintain complete and detailed records of the patient’s health care plans and progress.

• Perform daily patient rounds, coordinating care with the surgical team, nursing staff and ancillary team members.

• Prepare patients for discharge by providing education to them and their family members. Coordinating follow-up care with outside facilities and ensuring that appropriate referrals are in place for in-home supportive services.

• Provide care in the Ambulatory Outpatient Practice when needed, including wound care and urgent care needs.

• Providing care throughout various outpatient settings (Respiratory Illness Clinic, Surge Clinic) and in various roles (RN, APP) as needed.
