I have been a Registered Nurse since April 2007, in the years that have followed my licensure, I have gained valuable experience in the nursing field. In May of 2013, I graduated the University of Houston-Victoria with a BA in Science/Nursing. I have held many positions as a nurse, however, the one that I have found to be most fulfilling is that of case management. I also have experience in working in the settings of medical/surgical unit, long term acute care/rehabilitation, charge nurse, case management, travel nursing, and wound care.

Medical Management Specialist I, Blue Cross Blue Shield- STAR Kids

· Point of contact for member’s/providers to assist in management of care of the individual that is 21 years old, or younger with complex/chronic healthcare needs. Help to navigate member’s benefits in finding providers that are in-network of the insurance carrier, authorizations/referrals in the benefits of the insurance, resources for meals/transportation, implementation of care plans, assessments/face to face visits with member’s when needed, discharge planning/follow-up care, LTSS services/DME needs. Required some travel to meet with member’s/family. December 2015-

June 2018

RN Case Manager, Accentcare-Texas Home Health

· Built on nursing assessment skills/knowledge, performed intake assessments to assess client’s needs, wound care, medication management, IV therapy administration, obtaining/clarifying/transcribing orders from providers, collect lab as ordered, collect vital signs, referrals/authorizations, coding, and marketing. Requires the nurse to participate in discharge planning, and build on management/delegation skills by managing a LVN. Required some traveling to visit with clients.

COMMUNICATION

I work well in a team setting, and individually. My assessment, and communication skills are strong. I have a good rapport, and understanding of the needs in the MRSA (south Texas) region, and Victoria area. I am able to speak, read, and understand Spanish language. As a nurse/educator I have come to learn, and appreciate the knowledge and experience of others. I learn from them, and then take their experiences and use it for my own personal growth, and the growth of others. I feel like an opportunity as a case manager affords me a chance to serve the under privileged, and underserved.

LEADERSHIP

I have held the position of charge nurse, management of a LVN while working in a hospital, and home health positions. I feel the most comfortable in my nursing profession, and have no issues with taking control of a situation in order to get things done. In most of my experiences as a nursing professional, I have remained calm/collected at times when others may have shown less of level head.
