Followed and implemented changes to plan of care to comply with Medicare and all insurance carriers. Worked with patients to schedule sessions that complied with their guidelines.

Excelled at communicating to multidisciplinary team and staff to provide quality care and implement plan of care for patient.

Created and implemented program, “General Overview of Transfer Safety Tips and Techniques”, to instruct certified nursing assistants in large groups of over 40 people, and smaller groups of 1-5 people. Program format consists of transfer training, proper body mechanics to decrease injury to staff members, weight bearing restriction demonstrations, and wheel chair maintenance regarding safety with patient care. This program that provides verbal, as well as hands on training is approved by assistant director of nursing.

Conducted family training sessions involving patients, as well as family members/caregivers regarding proper techniques of maintaining and implementing functional mobility, home exercise programs with handouts, as well as safety awareness training to prepare patient and family member/caregiver for discharging to home environment. These sessions prepare all involved for patient’s level of assistance that will be required at their current level of function at discharge with emphasis on safety.

Ascension Health (Formerly Presence Health) Mokena, IL October 2015 - June 2018

Implemented a program entitled “General Overview of Transfer Safety Tips and Techniques”. The program served to train certified nursing assistants in safety by using demonstration and hands on training consisting of transfers, proper body mechanics to decrease staff injury, as well as weight bearing restriction compliance.

Coordinated and evaluated CNAs facility wide in yearly skill requirements.

Was a clinical instructor for PTA students.

Performed home evaluation visits for the associated assisted living facility to evaluate a patient’s eligibility to either return to assisted living independently, or recommendation of supervision.

Renewal Rehab, Skokie, Il April 2012 - January 2013

Performed home visit assessments to assess patient’s living arrangements following discharge to assure safety, and determine patient’s level of assistance required.

Worked hand in hand with multidisciplinary teams and facility teams creating, implementing, and providing plan of care for patients.

Provided patients with therapy using massage, therapeutic exercise, balance training, and therapeutic modalities, such as electrical stimulation, ultrasound, and diathermy.

Worked with prosthesis team to help patients with fitting of prosthesis, and ongoing prosthetic rehabilitation.

Worked as on-site clinical instructor for physical therapist assistant students.

Elmhurst Memorial Healthcare, Elmhurst, IL August 2008 – February 2012

Worked in an acute and sub-acute hospital setting, following plan of care to provide physical therapy services to patients in ICU, cardiovascular unit, stroke, neurological disorders, post-operative orthopedic, and general health deconditioning.

Presented bi-monthly seminars regarding total joint replacement for the Elmhurst community and Elmhurst Memorial Healthcare orthopedic patients. These patients, referred to me by Elmhurst Memorial Healthcare orthopedic surgeons, were educated and prepared for joint replacement.

Worked as onsite clinical instructor for physical therapist assistant students.

Was nominated and acknowledged for Elmhurst Memorial’s PRIDE award for outstanding presentation regarding in-service presentation to rehab staff and nursing staff.
