Aesthetics company owner; Professionally trained to provide services and treatments such as intradermal Botox, fillers, superficial skin lesion removal, biopsies and Sclerotherapy. Additionally; professionally trained to provide custom diets, and exercise programs for weight loss. I also have provided group sessions for motivational change including for diabetes, weight loss and smoking cessation.

Responsibilities: Occupational health, primary care, urgent care services are provided to all populations. Injury and urgent care treatment for head to toe health evaluation of many conditions. I provide preventative services and treatment following USPSTF recommendations. DOT Medical exams, pre-employment, return to work, and preoperative exams are provided. I am Certified to provide Logistics Health Inc, (LHI) exams for Military Health Readiness, Veterans Compensation/Pension claims, and Federal Railway Administration exams. I am credentialled with the MN HealthCare Insurance Consortium.

This company was previously named US Health Works then renamed after being acquired by Concentra.

Titles held: Clinic Leader, National Trainer with US Health Works.

Title changed to a Mid-Level Primary Provider with Concentra acquisition.

When acquired by Concentra my position consisted of the same responsibilities.

Responsibilities: Occupational health services; injury care, pre-placement, return to work, DOT medical exams, and urgent care services for all age populations. Injury and urgent care services included laceration and wound evaluation, local and or digital blocks for repairs. Frequent simple to complex suturing skills. x-ray ordering/reading for splinting sprains and fractures. Ordering and review of chest x-rays for illness or injury care, diagnostics for conditions such as pneumonia or ruling out TB, or Silicosis. Ordered and reviewed diagnostics for conditions such as lead toxicity, blood borne pathogen exposures, respiratory questionnaire and review for silicosis and asbestos exposure, evaluation and care for bites from animals and humans. Abscess evaluation, I&D and follow up care referrals. Pain management procedures such as large or small joint aspiration, joint injections, trigger point injections, ordering and communicating frequently with physical therapy within the clinic. Evaluations of rash, lesions, procuring skin, fluid or tissue biopsies. Treatment of reactive airway, asthma, allergic reactions. I have had training and frequently used a slit lamp for eye injuries. I was also a resource for difficult phlebotomy draws. I also frequently ordered and reviewed immunization statuses. Triaging patients to the most appropriate level of care, provided assist and transfer communication to higher level of care settings as necessary to promote safety and best care.

Women’s health exams, including Pap-smears, manual exams and STI lab testing.

All or any of the skills that I possess have been demonstrated with instruction for other providers including APRNs, PA and Medical doctors within US Health Works and Concentra as asked. Often US Health Works and Concentra existing patients were sent from other locations for eye injury care, women’s health, ABI testing, and for specific procedures such as necessary evaluation with a slit lamp, joint aspiration/injections, trigger point injections with and without ultrasound guidance. Nursing and MA skills and procedures such as EKG, phlebotomy, immunizations, ABI testing demonstration/Instruction have been provided as asked for all MN locations. I also mentored and precepted APRN and PA students.

August 2017 to February 2018 Part-time

Title: APRN, Urgent Care Provider.

Responsibilities: Provided urgent care services for all populations. Eye conditions and injuries, upper respiratory and sinus illnesses, cough, lung illness and conditions, and evaluation of fever, skin rash and lesions, wound evaluation and care including laceration, repair simple to complex closures. Fracture care including X-Ray, splint and casting. Focused attention given to antibiotic and opioid over-use and followed good steward recommendations to prevent over-use complications related to ordering un-necessary antibiotics or opioids. Ordered referrals to all specialties as needed. I was a mentor and preceptor for APRN and PA students from the first month and until leaving.

I left this position because US HealthWorks/Concentra offered me a full-time position.

Augsburg University, Minneapolis MN

June 2017 to August 2018

Title: Clinical Director for Family Nurse Practitioner/DNP program. This position was considered a part-time position.

Responsibilities: Following Augsburg’s mission and goals I procured new clinical locations/sites for FNP students, created new and extended previous affiliation agreements with hospitals and clinics throughout the twin cities, Minnesota and Wisconsin. This position required much more time and consideration than could be provided in a part-time appointment. I very much enjoyed this position however was not able to continue to work full-time in my day to day job and fulfill all of the needs that this position would require in the future. I provided an intent to resign in 4 months letter to the program director. I did fulfill the position needs for the entire school year including an additional summer semester and provided a comprehensive hand-off of responsibilities to the Nursing Program Director. Title: Clinic Medical Director. Nurse Practitioner in Private Community based clinic.

Responsibilities: Creation of a multi-disciplinary care approach to health and wellness for individuals, and entire families/populations of all ages. Primary care provided for a moderate sized panel of patients with various co-morbidities, offered specialty services in a primary care environment. Examples: joint injections, skin procedures, biofeedback, pain management, bioidentical hormone therapy, sexual and reproductive health services for men and women. Title: Nurse Practitioner in primary care, internal medicine and women’s health clinics. Each of these opportunities has improved by confidence and practice by supplementation of my knowledge and skills. I was the Chair of the Advanced Nursing Practice Council for the VA hospital and clinics which consisted of providing professional support, and mentorship of 174 APRNs. I have had the pleasure of influencing other APRNs and modeling ideal leadership. I also have been an instructor and a Nursing Core and Spinal Cord Injury Educator for the MN nursing departments before becoming a Nurse Practitioner within this same medical facility and healthcare community.

A systems change project was completed by me; this included staff and provider education and care improvement for an underserved, marginalized population within the VAMC. I have been a source for consultation for other providers in the care of the Transgender Veterans, working side by side with a newly established multidisciplinary team which has made significant improvements.

I was previously a nursing clinical instructor and spinal cord educator at the MVAMC. My duties included: curriculum development, presentation and instruction within a classroom and providing demonstration and mentoring for staff. I have developed, presented and edited curricula for Twin Cities Hospital Consortium, (TCHP) listed below.

Faculty Instructor, I have presented to 5 affiliated twin cities hospitals in MN. Preparation for courses included editing and developing course curricula for health professionals. Course presentations were provided several times a month and some quarterly. Courses included: Intro to Spinal Cord Care and Rehabilitation; this is a two-day intensive course; Cardiovascular Assessment; and I have presented as a substitute for multiple other course faculty members presentations.

Whole family nursing care and teaching were essential and rewarding. I also scrubbed and circulated in the OR for OB cases.

I started as a Case Manager and then was promoted to Director of Staff Development and Human Resources. As a Case Manager I provided skilled nursing visits including IV therapy, wound care, cardiovascular and health assessments. We provided Medicare certified home care, and employed 320 employees. Patients received excellent individualized care from highly trained individuals. Duties as Director of Staff Development included curriculum development teaching and mentoring staff. I developed a system for recording and documenting staff licensure status, various other educational requirement calendars.

Staff Nurse on general care, telemetry, float pool, labor and delivery and OB units full time for the first year and then part-time when I joined on at Compassionate Home Care. I learned a tremendous amount caring for patients and families here.

I attended the New Graduate RN program offered by the hospital. I was staffed on general care, medical surgical and float pool units and as a trouble shooter for starting difficult IVs within the float pool. Working here I gained organizational and assessment skills. I attended the hospital critical care courses and then was staffed on telemetry floors.
