Title changed to a Mid-Level Primary Provider with Concentra acquisition.

When acquired by Concentra my position consisted of the same responsibilities.

Responsibilities: Occupational health services; injury care, pre-placement, return to work, DOT medical exams, and urgent care services for all age populations. Injury and urgent care services included laceration and wound evaluation, local and or digital blocks for repairs. Frequent simple to complex suturing skills. x-ray ordering/reading for splinting sprains and fractures. Ordering and review of chest x-rays for illness or injury care, diagnostics for conditions such as pneumonia or ruling out TB, or Silicosis. Ordered and reviewed diagnostics for conditions such as lead toxicity, blood borne pathogen exposures, respiratory questionnaire and review for silicosis and asbestos exposure, evaluation and care for bites from animals and humans. Abscess evaluation, I&D and follow up care referrals. Pain management procedures such as large or small joint aspiration, joint injections, trigger point injections, ordering and communicating frequently with physical therapy within the clinic. Evaluations of rash, lesions, procuring skin, fluid or tissue biopsies. Treatment of reactive airway, asthma, allergic reactions. I have had training and frequently used a slit lamp for eye injuries. I was also a resource for difficult phlebotomy draws. I also frequently ordered and reviewed immunization statuses. Triaging patients to the most appropriate level of care, provided assist and transfer communication to higher level of care settings as necessary to promote safety and best care.

Women’s health exams, including Pap-smears, manual exams and STI lab testing.

All or any of the skills that I possess have been demonstrated with instruction for other providers including APRNs, PA and Medical doctors within US Health Works and Concentra as asked. Often US Health Works and Concentra existing patients were sent from other locations for eye injury care, women’s health, ABI testing, and for specific procedures such as necessary evaluation with a slit lamp, joint aspiration/injections, trigger point injections with and without ultrasound guidance. Nursing and MA skills and procedures such as EKG, phlebotomy, immunizations, ABI testing demonstration/Instruction have been provided as asked for all MN locations. I also mentored and precepted APRN and PA students.

Title: APRN, Urgent Care Provider.

Responsibilities: Provided urgent care services for all populations. Eye conditions and injuries, upper respiratory and sinus illnesses, cough, lung illness and conditions, and evaluation of fever, skin rash and lesions, wound evaluation and care including laceration, repair simple to complex closures. Fracture care including X-Ray, splint and casting. Focused attention given to antibiotic and opioid over-use and followed good steward recommendations to prevent over-use complications related to ordering un-necessary antibiotics or opioids. Ordered referrals to all specialties as needed. I was a mentor and preceptor for APRN and PA students from the first month and until leaving.

I left this position because US HealthWorks/Concentra offered me a full-time position.

June 2017 to August 2018

Title: Clinical Director for Family Nurse Practitioner/DNP program. This position was considered a part-time position.

Responsibilities: Following Augsburg’s mission and goals I procured new clinical locations/sites for FNP students, created new and extended previous affiliation agreements with hospitals and clinics throughout the twin cities, Minnesota and Wisconsin. This position required much more time and consideration than could be provided in a part-time appointment. I very much enjoyed this position however was not able to continue to work full-time in my day to day job and fulfill all of the needs that this position would require in the future. I provided an intent to resign in 4 months letter to the program director. I did fulfill the position needs for the entire school year including an additional summer semester and provided a comprehensive hand-off of responsibilities to the Nursing Program Director. Each of these opportunities has improved by confidence and practice by supplementation of my knowledge and skills. A systems change project was completed by me; this included staff and provider education and care improvement for an underserved, marginalized population within the VAMC. I have been a source for consultation for other providers in the care of the Transgender Veterans, working side by side with a newly established multidisciplinary team which has made significant improvements.

I was previously a nursing clinical instructor and spinal cord educator at the MVAMC. My duties included: curriculum development, presentation and instruction within a classroom and providing demonstration and mentoring for staff. Faculty Instructor, I have presented to 5 affiliated twin cities hospitals in MN. Preparation for courses included editing and developing course curricula for health professionals. Course presentations were provided several times a month and some quarterly. Courses included: Intro to Spinal Cord Care and Rehabilitation; this is a two-day intensive course; Cardiovascular Assessment; and I have presented as a substitute for multiple other course faculty members presentations.

Patients received excellent individualized care from highly trained individuals. Duties as Director of Staff Development included curriculum development teaching and mentoring staff. I developed a system for recording and documenting staff licensure status, various other educational requirement calendars.
