As the organization’s only Nurse Practitioner, I was responsible for completing Medicare recertification on all patients enrolled in the county-wide Hospice program.
Support for physician staff for Hospice as well as Palliative patients. This included both patients in the field and in the organization’s Hospice facility.
On behalf of the organization, I developed a system by which each patient could be monitored for visits needed in the final days of life, to insure the appropriate care and comfort of the patient, as well as to guarantee that the federal metrics are met for compliance and payment.

Developed and managed a book of new as well as existing patients for a multiple location, internal medicine private practice. This included total coordination of healthcare services, diagnostics and treatment. This practice was servicing the needs of the Medicare/Medicaid patients of the area.

Hired to develop a program in which “high-dollar” patients would be serviced by an NP in the field (in collaboration with their PMD) as a cost saving measure. This program led to an overall decrease in LOS from 18 days down to 15 days, as well as substantially decreased return admissions.
NOTE: Near the end of my tenure with Esse Health, I was recruited by one of the member physicians to take over their 400+ patient practice, due to the physician’s terminal diagnosis.

Responsible for the total care of the hip/knee replacement patient from their post-operative admission to the recovery room until they were discharged to home, rehab or SNF. I managed a wide variety of conditions. During my tenure with this department, I reduced the patients’ length of stay from 3.5 days to 2.2 days.

Provided direct supervisory responsibility for the entire 1700 bed hospital. I developed new care protocols between administrative, patient care and clinical staff.
Staff Nurse – Emergency Department – Level One Trauma Center (American College of Surgeons rating – July 1999 – March 2003, interim March 2006-June 2006.
Duties included initial and ongoing assessment/care of the patient from admission to disposition. I worked in both the Emergent area as well as the Trauma/Critical Care area and was involved in formulating ground-breaking protocols in the ER.
