COMMUNITY RESOURCE DEVELOPER TRUMAN MEDICAL CENTER HOMELESS SUPPORT PROGRAM
Went to homeless shelter to locate SPMI patients and invite them to participate in homeless program, supported with medication assessment by a Truman Medical Center Psychiatrist. Psychoeducational support. Assisted patients with getting medication and compliance. Assisted in locating permanent housing and in obtaining social security benefits and Medicaid for support. Crisis intervention. Was a member of the multidisciplinary treatment team, advocating for the patient.
