Circulating soluble Selectin as diagnostic marker for early identification of pregnant women at risk for development of preeclampsia.

Conclusion:

Preeclampsia is the one of the most serious complications in pregnancy, the cause of it is still unknown but many theories try to explain its pathophysiology ; one of them suggest the role of adhesive molecules in the pathogenesis of preeclampsia .

Adhesive molecules are important in the development and maintenance of inflammatory and immune responses and leukocyte-vessel wall interactions. Three of adhesive molecules exist on the surface of leukocytes: immunoglobine super family, Selectins and Integrins.

Endothelial Selectin is found to increase in cases of preeclampsia this may play a role in pathogenesis of preeclampsia.

-Hypertensive disorders in pregnancy compromise a major cause of maternal and fetal mortality and morbidity.

-Early detection of preeclampsia facilitate proper planning of management and sharp timing of delivery prior to occurrence of serious squeals.

-Among parameters for early detection are clinical, mechanical, biochemical ad lastly ultrasonic Doppler wave study predictions.

-Transvaginal pulsed color Doppler ultrasonography allows noninvasive evaluation of uterine circulation.

-Uterine artery is considered the best vessel to reflect the impaired uterine perfusion during the early periods of pregnancy.

-Assessment of Selectin levels in the first trimester is highly predictive for early detection of cases that mostly developed preeclampsia but still in need for

a complementary predictive tests to strengthen the predictive power for preeclampsia.
