This school nurse position was attained through a Federal Grant offered through MDHHS, Michigan Department of Health and Human Services. My professional duties consisted of providing updated Covid-19 clinical guidance to the school administrators, teaching staff, and parents of students who fell ill to Covid-19 while attending Avondale School District. Covid-19 testing was also performed onsite by the school nurse within a designated clinic in the administration building between the hours of 07:00am-3:00pm daily Monday through Friday. The school nurse/ HRA (Health Resource Advocate) distributed rapid antigen tests to all school buildings several times throughout the school year, particularly during school breaks, and holidays, when the congregation of staff and students with their families and friends was anticipated, to help mitigate the spread of Covid-19. The school nurse documented and was the mandated reporter to the Oakland County Health Department of Covid-19 cases and other communicable diseases such as Monkey pox, Influenza A & B, RSV, Hand Foot Mouth Disease, Measles, etc. The school nurse educated the community and encouraged childhood vaccination of Covid19, Influenza, RSV, and Measles where appropriate, when receiving updated notifications from our district and state nurse consultants. The school nurse collected data quarterly and reported this data to MDHHS of the Covid-19 cases, and other clinical findings, including certifications attained by the school nurse such as Mental Health First Aid, Seizure & Asthma Training, Diabetes Training, Legal Implications and Medication Administration of Students with Medical and Chronic Conditions, MERT/CERT Training in Schools, Stop the Bleed Certification, and participated in assisting the district nurse with one of their elementary schools, attain the MI Heart Safe Designation on May 15,2023. I also helped support the district nurse take care of students with special needs requiring tube feeding, and ureteral catheterization. I worked primarily within the Central Administration office, school year 2022-2023. I provided clinical support for the students with Special Needs during their training for the Special Olympics held at a designated Gross Pointe track and field in May 2023.
The school nurse also provided summer school clinical support at Oak Park Academy HS in June 2023. The school nurse resumed the school year 2023-2024 at Avondale School District providing clinical support working with students from Avondale Middle School. The nursing duties involved assessing middle school students and their physical, mental and socioemotional needs, referring them to their pediatricians or primary care providers, to the guidance counselors, and to the dean of students, when there were disciplinary issues to be addressed. I also referred the parents to the Urgent Care or ER, when students presented with acute orthopedic injuries, strep throat infections, persistent vomiting, intractable diarrhea, severe abdominal pain, allergic reactions, exercise induced asthmatic attacks, and upper respiratory infections, severe PMS symptoms, potential ear infections w/ear pain, and students that presented with dehydration and complaints of lightheadedness or vasovagal syncope. Students that displayed signs of anxiety and avoidance coping mechanisms were referred to mental health care providers within the community for medication and cognitive behavioral therapy. This year, the principals have actively involved the school nurse in providing health education regarding the health hazards of vaping, alcohol and marijuana in the youth. The school nurse continues to update the school district with CDC guidelines regarding Covid-19 and any other potentially threatening communicable diseases such as measles and continues to be the point of contact for Avondale School District when staff or students attending the school district require clinical guidance regarding Covid19 and any other communicable diseases. They have placed me in their newsletter as their point of contact. I am a first responder for Avondale School District as I am actively involved in their medical emergencies. My professional duties consisted of assessing the health needs of general education students from K-5th grade and POHI students, also known as partially/or otherwise health impaired students. These students have diagnoses with spina bifida that are wheelchair bound and require urinary catheterization secondary to neurogenic bladders and require assistance with toileting needs, students with seizure/epilepsy disorders requiring frequent neurological monitoring to help prevent seizure attacks, and the nurse would administer Klonopin according to the pediatrician’s medical orders in the event of a myoclonic tonic seizure or Grand Mal seizure, students with tracheostomies, requiring frequent suctioning, and prn nebulizer treatments with Albuterol, frequent oxygen saturation monitoring, and students with oral impairments requiring gastric tube feeding, intermittent or continuous tube feed as ordered by their respective primary care physicians/pediatricians. The nurse also assisted with cognitively impaired students with Autism, monitoring their safety while participating in their school activities. The school nurse also assessed the common health needs of general education students such as c/o fever, c/o sore throat, nausea/vomiting, abdominal complaints, screening for Covid19 symptoms, advising the parents according to the updated CDC guidelines. The nurse also assessed students with eye signs and symptoms, demonstrating potential bacterial
& or viral conjunctivitis, sending them home to mitigate cross contamination with other students and staff, and addressed trauma issues such as falls with extremity fractures, nose bleeds, and performed nursing care according to Michigan Standards of Best Practice, and in accordance with the individual school policies/protocols, as advised by the CDC. The school nurse contacted the parents and/or legal guardians to communicate their children’s health complaints and advised them to follow through as they deemed appropriate with medical evaluation to assure the speedy recovery of their children, and to assure their safe and healthy return to school, avoiding unnecessary loss of school time.The school nurse was accountable to the parents communicating the incidents that occurred within the school environment and was answerable to the school principal and the agency supervisor responsible for recruiting the school nurse . My professional duties consist of assessing, and treating a diversified population of high school students. The student cultures include primarily Hispanic students, White American, African American, Middle Eastern students and students of European descent. The school nurse is actively involved in assessing, formulating action plans for students with ailments such as asthma, juvenile diabetes, seizure disorders, and mental health/anxiety disorders, such as manic/depressive disorder, OCD, and special needs students with autism, and ADHD. In light of the Covid-19 pandemic declared in March of 2020, the Detroit Public School System in close partnership with Henry Ford Health System and Wayne State University School of Medicine, actively employed school nurses to assist with Covid-19 screening, dismissing symptomatically ill students for Covid-19 testing, follow-up with their testing to maintain their PCR test results on file with the school, and to report any positive tests to the Detroit Health Department weekly. The school nurse also works closely with the school Principal, Assistant Principals, school deans, secretarial and custodial staff to assist in mitigating the spread of Covid-19 within the schools.
