skill set, including conducting physical assessments, developing

patient care plans, administering medications and treatments, and

handling sterile procedures, tracheostomy care, and specimen

collection and analysis. With my proficiency in monitoring

respiration, blood pressure, and blood glucose levels, I am able to

diagnose and manage patients effectively.

I have a proven track record of collaborating with interdisciplinary

teams of healthcare and social service providers to address patients'

needs through effective intervention and care planning, and am

committed to educating patients and their families on medical

diagnoses, treatment options, chronic disease self-management, and

wound management.
