The primary goals of the Aetna Care Coordination program are to improve care coordination; increase access to home and community-based care; increase access to natural supports; equip children, adolescents, and families with the skills needed to manage their care; improve youth and family outcomes; and reduce the cost of behavioral health social services. In this role I worked primarily within Promoting Active Connections. This involved making outbound calls to members eligible for the service to ascertain interest. I completed assessments and made referrals to active Care Coordination, as well as making referrals to community resources according to need indicated by assessments and member or guardian request.
I lived in Hawaii from November 214 until November of 2021. I moved to McAllen, Tx once I left Hawaii and was unable to find employment there. But I was offered the position back here in Ohio and returned to my home state.

CRISIS THERAPIST

The Crisis Therapist fields on-call crisis outreaches for the Crisis Line of Hawaii and maintains a load of cases of thirty-day duration for follow up and linkage to systems and long-term supports. Services provided directly and indirectly by the Crisis Therapist include psychiatric/ therapeutic intervention and referral, linkage to financial benefits and housing resources, as well as support directly to the individual and their family, if relevant. This position requires extensive interaction with local systems and frequently is called to meet new clients in the emergency room or on discharge from psychiatric hospitalization.

The Hawaii Island Home for Recovery provides transitional housing for adults who have been homeless and works with those individuals to find independent long term housing arrangements. The Case Manager and the placed individual work to develop an Individual Service Plan to address roadblocks, such as addiction, mental or physical health concerns, or lack of finances; to the individual obtaining long-term safe housing. The Case Manager and the resident work with community agencies and resources to facilitate these goals. The Case Manager also works with residents to resolve day to day issues and/or conflicts.

CASE MANAGER/ OUTREACH COUNSELOR
May 2019 to June 2021(two years, one month)
Case Managers coordinate and supervise skill building visits between parents and their children in the temporary custody of Children’s Service and report progress in those visits. The Case Manager also maintains the physical and electronic records. Outreach Counselors work with a caseload of eight to twelve parents or couples to do counseling on issues related to barriers to reunification of the children with their family of origin. The Outreach Counselor also provides case supervision to the direct providers of skill building visits with the children and parents. The Outreach Counselor provides regular reports on case progress and interacts with parts of the larger community case team for the youths and the adults, including behavioral and physical health providers, and the courts. There was a strong emphasis on use of motivational interviewing
