Volunteering or employment with American Red Cross ("RED CROSS") requires you to obtain a background check through ASURINT., a national background screening company. In order for ASURINT to deliver electronically a background report to RED CROSS, you will need to give consent to (a) receive and respond to information in electronic form and (b) for RED CROSS to obtain a background check through ASURINT. I understand that any consent given here applies to the electronic transactions related herein to my volunteer or employment position with RED CROSS and the order for a background check through ASURINT. I understand that if I choose to withdraw my consent to electronic transactions related to my position with RED CROSS and the order for a background check through ASURINT, I must submit a written request to: ASURINT Compliance Department, P.O.
