Case Management

Conducts client intakes, needs, and patient psychiatric social assessments, conducts risk assessments to identify clinical/case management needs, and establishes individualized treatment plans to address those needs.

Coordinates with the mental health and psych assessment team, Qualified Health Professionals (QHP), and the Multi-Disciplinary Team (including psychiatric NP and medical staff) to determine the correct level of care for each client, including Medication-Assisted Treatment (MAT), harm reduction, and mental health and family navigation.

Clinical Approach

Utilizes a person-centered methodology, Motivational Interviewing, Cognitive Behavioral Therapy, and tactics approved by OASAS.

Provides individual/group counseling to assist patients with recognition of triggers to substance abuse and develops appropriate coping mechanisms to prevent recurrence of use, and recovery.

Educates patients in all aspects of treatment, corresponding health issues, and steps to recovery, while actively participating in client empowerment philosophy and practice.

Maintains thorough client documentation (per OASAS requirements) in the 10e11 documentation system for each instance of client contact. Maintains confidential and appropriate case management system by keeping a record of input group notes, safety plans, treatment plans, and continuing care. Strictly adheres to HIPAA and 42CFR guidelines and confidentiality rules and regulations.

Cross Collaboration & Coordination

Coordinates care with the Department of Social Services, multiple county courts, and jail personnel (probation and parole) and supervises urine screen samples for lab analysis as required.

Provides ongoing coordination with client's attorneys, CPS, and the court system providing weekly reports to Drug Treatment Court for all clients on caseload.

Possesses access to the Impaired Driver System NYS and NYS Department of Motor Vehicles for individuals with license restriction or suspension and charges such as DWI, DUAI, etc.

Served as sole counselor for this transitional facility. Counseled a group of 15 homeless individuals with a minimum of 30 days of sobriety. The group comprised of 13 men and two women, military veterans, and civilians.
