ACKNOWLEDGEMENT AND AGREEMENT

By signing below, I acknowledge that I am not agreeing to or required to purchase any goods or services. By signing below, I acknowledge the following and agree to be bound by the following terms:

● I acknowledge that PLM is not a law firm, has not provided me with legal advice, or otherwise engaged in the practice of law. PLM provides a matching service for legal professionals based on my geographic location, and PLM does not recommend any matched legal professional. I understand that PLM may provide marketing and administrative services on behalf of legal providers or other entities, and that no attorney-client relationship has been formed based on my interaction with PLM.

● I represent and warrant that the information that I have provided to PLM is true to the best of my knowledge and that I will indemnify and hold PLM harmless to the extent that PLM is subject to a lawsuit, legal action, damages, or loss associated with my knowingly providing false information.

● I have requested that matched legal providers and their affiliates, attorneys, representatives and agents may contact me via telephone calls, text messages, and email to further discuss my potential legal claim and related issues.

● I requested and provided prior express written consent to be contacted by PLM to call and text me, including using automated technology and prerecorded voice, regarding potential legal claims prior to my receiving a call or text message from PLM.

● I hereby release PLM from any claim, suit, or action under the Telephone Consumer Protection Act and other applicable laws arising from, in connection with, or related to the services provided by PLM and my interaction with PLM. I hereby covenant that I will not file a court action or other legal proceeding against PLM for any claim that I was aware of or arose prior to my execution of this Acknowledgment and Agreement.

● JURY AND CLASS ACTION WAIVER. I waive all rights to a trial by jury in any legal action against PLM, and I will not join or consolidate claims against PLM in court as a representative or member of a class or in a private attorney general capacity.

● I agree to defend, indemnify, and hold PLM and its affiliates, subsidiaries, successors, assignees, owners, directors, officers, employees, contractors, representatives, and agents harmless from and against any and all claims, actions, and losses (including, without limitation, reasonable attorneys’ fees and expenses) arising out of or related to my breach of this Acknowledgement and Agreement, willful misconduct, or gross negligence.

● I have authorized PLM to provide my information to legal providers or other entities in connection with my request for legal services and PLM is authorized to collect, maintain, and use my personal information pursuant to its privacy policy, which I agree to and acknowledge.
