Case Management skills to include hypertension, diabetes, and other chronic disease management skills such as assuring that the patient is up to date on all labs, have appropriate supplies, as well as keeping them current on the latest disease management and education. Also, I call labs, X-rays, MRI's (etc) results in patients after the provider has reviewed. If results are abnormal or sometimes even if they are normal I notify pt of provider's restrictions/recommendations, ex..medication changes, additional tests which may need to be ordered, referrals which may need to be placed, (ex.broke bone = orthopedic referral) etc. If referrals are needed I placed the pt's referral as well as often assist them with scheduling an appt. to include continuity of care with particular specialist pt may have been previously receiving treatment from or currently being treated by, which would be a referral extension. I also provide assistance to pt's with their MEB & RILO processes and appointments which involve a great deal of research through the patient's an actual paper chart or obtaining specialist notes, as well as reviewing AHLTA records. Telephone and in-person assessment..for example: I triage patients as needed whether they come in with an "eagle bite" from the flight line, walk in complaining of chest pain or call in complaining of a sore throat, ect., I often save the clinic money through my assessment and being about to provide patients with home care or evaluate that patient needs to be seen either through the provider or referred to urgent care or ER as needed. Knowledge of a variety of medications used in patient treatments, the desired effects, side effects, and complications of their use as well as the accurate administration of the particular medication. Provide patients and their families education for home care, medications, lab results, X-ray results, referrals and referral processes, etc. Handling, assisting, and or documenting emergency situations as necessary. Assisting meditechs as needed to ensure safe and competent care is provided to patients. Excellent AHLTA & CHCS skills and knowledge. Very skilled and competent when dealing with TRICARE. I received great experience in medical, emergency, telemetry, intensive care, ortho-neuro, and pediatrics.
