Health Specialist:

My job as a health specialist is to contact Medicare/Medicaid members and their plan benefits. To encourage them to get their yearly screenings such as (flu shots, annual wellness checkups, mammograms and so forth) and to also discuss their health plans and benefits. I also offer them resources within the community to help assist their needs such as: doctor appointments, transportation, food resources, meals on wheels, pharmacies, etc. I also discuss blood pressures, cholesterol and diabetes with members and ways we can prevent these from occurring. Their medications are discussed and encouraging the member to refill regular and attend doctor appointments regular as well. If needed I can refer them to a case management if they require further assistance.

Behavioral Health group (BHG):

Case management:

My job as a case manager was work one-on-one with patients with addiction. I provide resources within the community that the patients could benefit from as far as their daily needs. I conducted treatment plans, intake assessments and needs assessment on each client as well. A note is documented at each visit which is twice per week or more per patient.

Kentucky River Community Care

Program facilitator: 2014/2020

My job as a program facilitator (supervisor) included monthly reports to the state, reported CDO numbers monthly to Medicaid, did recoupments 1-2x yearly, administrative monitoring, held meetings and trainings with the 9 employees and new hires. I worked closely within the community, with families/clients and Medicaid.

Kentucky River Community Care: 2010 - 2014

Case manager:

My role as a case manager was to conduct a face-to-face home visit within the home of the clients, conduct monthly telephone contacts with participants or guardians, initiating and updating yearly treatment plans, managed clients yearly ($40,000) yearly state budget for services, conducted annual re-assessments and yearly paperwork and submitted to Medicaid for approval.
