As well as the development of case management approach that would be most beneficial to the applicants. I also developed SOP’s that I utilized to train all new incoming staff as well as senior staff utilizing the training tools and SOP’s I developed for future training and development.
• Managed cases of families living in temporary shelters and ensured services for 6 months after finding permanent housing program, but I made sure to gather information and develop systems that would provide the most efficient approach to long term housing. Such as utilizing not only feedback from the participants but working within the community to formulate case management approach that resulted in almost 60% improvement rate in housing of homeless families.
• Expert in the coordination of services that include mental health resources (such as crisis workers, therapists, and substance abuse counselors). I also was able to further refine and establish the processing and understanding of obtaining benefits as well as tenancy skills, and client advocacy resources for clients. I would then review Child welfare services reports and documentation and speak with Guardian ad litem assigned to the case for further information.
• Once all information is gathered, I would create a final report and present this to senior staff for final review.
• Once report is finalized, I would then present these findings to the Family Court hearing which included: family history, environment, relationships, police or previous court reports and needs/perspective of minor/child in question.
