Duties and Responsibilities include - Contacts assigned consumers and documents; contacts according to established SCLARC Standard Operating Procedures (SOPs) and policy and completes required documentation. Prepares case transfers as needed. Attends all unit, division, supervision, staff and in-service training meetings as required. Attends all Planning Team meetings at primary programs and any other consumer related special meetings/hearings as required.
