Carolinas Healthcare System/Sisters of Mercy Grant.Grant case manager:Managed a CAP/DA waiting list over two years long. Met with potential clients and caregivers to share resources available as they waited for CAP/DA assessment, provided contact information and other assistance accessing community agencies that offered interim support for waitlist clients. (September 2012 to March 2013.)
Person Centered Partnerships. Mental health case manager: Created and tailored Person Centered Plans for the specific needs of individual clients, updated electronic medical records, connected with community resources such as food pantries, affordable housing, and medical clinics, provided counseling and support for a caseload of thirty-eight clients. (April 2011 to April 2012.)
Daymark Recovery Services. Community support professional: Managed a caseload of eight clients with intensive needs and at risk for institutionalization. Connected to community resources such as housing, vocational rehabilitation, psychosocial rehabilitation, and food and nutrition programs.
