Worked with adults who ranged from handicap, to emotionally developed intellectual individuals. Some of these adults were older and aging. Each of the individuals had care plans, which were developed and gone over in detail with the care team as well has the patient and patient support, if any. There was an older gentleman who had fallen and broke his hip, although, he was in pain, he did not want to go to physical therapy. The client knew his care plan addressed the care he needed to live alone. He also was aware he had to go to PT in order to live at home. A motivational tool was suggested; we could grab a pop or ice-cream on the way back home. We, did this continuously, throughout the 12 weeks of PT. Indiana Juvenile Justice Task Force-Indianapolis, IN May 1999 to August 2022
Worked closely with the juvenile probation department and the office of Family and Children, in Winchester, Indiana. Individuals were accompanied to court when they needed to be there. At, all times, there were a total of ten individuals in the program. These individuals could either be male or female. Upon entering the program, an intake and an assessment was completed in a timely manner, and presented to the team. At this time, a case management plan was completed with the individual present. Adolescents attended substance abuse groups, if they needed it and or anger management groups. All of these groups addressed the root causes of the underlying problem (s). Both cognitive behavior and behavior therapy were often the therapeutic choices. Individual and family therapy were designed to meet one time per week.
While in different groups individuals worked on empathy, social skills (by going out to eat), and school behavior skills.
Supervised a team of 4 adults. These adults where located in different areas in Indiana. We would discuss how, if any restrictions they felt. Especially, while performing the job they needed to do. The adolescents treatment plans were discussed also.
When it was time for discharge, all individuals were to meet the criteria for discharge. Follow up documentation on the individuals were documented in three -month intervals for a year. We, found, 98% of the individuals were doing well.

Organized and supervised the adolescent behavior unit in a residential facility. The adolescents who were placed had dual diagnoses. The behavior unit held 12 males and 6 staff. Intakes were completed at entry into the program. Assessments were completed and at this time both were discussed with the team.
Residents participated in group therapy, which focused on substance abuse, behavior issues, such as anger, poor social skills, and lack of coping skills. These groups would meet two times per week. Cognitive Behavioral Therapy worked best, at this time.
At discharge, all participates were included in the planning; including the individual and either the probation officer or DFC.
